Baby Cough: How to Calm a Dry or Wet Cough

Baby cough: coughing helps your baby clear their airways. If your baby coughs at night: dry or wet cough, cold, what helps, and when to call a doctor.

A baby with a cough usually has a cold. Coughing is a reflex that protects the airways, and it tends to fade on its own. You can help by rinsing the nose, offering fluids often, and keeping the room at 19-20 °C. No cough syrup before age 2. Under 3 months, or if your baby struggles to breathe, see a doctor the same day.

Hearing your baby cough at night can be worrying. Here is a guide based on France's national health insurance (Assurance maladie) and Santé.fr, to what causes coughing in toddlers, the simple steps that help, and the signs that mean you should call a doctor.

Why does a baby cough?

Because something irritates the throat or the bronchial tubes. Coughing clears the airways. It is a natural defense mechanism, a symptom rather than a disease, Santé.fr reminds us.

The most common causes of coughing are winter infections, starting with the common cold (nasopharyngitis), a viral infection. Other possible causes: bronchiolitis, laryngitis, flu, whooping cough, asthma, reflux, tobacco smoke or dust, or a small object swallowed the wrong way. Only a doctor can tell which one.

What's the difference between a wet cough and a dry cough?

If the cough doesn't bring up mucus, it's a dry cough: it comes from throat irritation. If it brings up phlegm, it's a productive cough, which helps clear mucus, Santé.fr explains.

In both cases, here is how to ease it in your baby: don't suppress the reflex. Help them clear the mucus and watch their breathing.

Why does a baby cough mostly at night?

When your baby lies down, mucus from the nose drips into the throat and keeps the reflex going. Air in a heated room also dries out fast. That's why a baby with a cold often coughs more at night.

A baby who coughs at night for several days in a row should see a doctor. So should a coughing fit that ends in vomiting or stops them from drinking.

How can you ease your baby's cough at night?

A saline nose rinse, frequent fluids, and a room at 19-20 °C are the key steps. Repeat them during the day. They won't shorten the illness, but they help your child feel better.

What helps at home?

Four steps, all recommended by France's national health insurance (Assurance maladie) or Santé.fr:

  • Saline nose rinse with saline drops several times a day, including once before bed. A clear nose helps soothe the throat (how to clear a baby's nose at night).
  • Offer fluids regularly such as breast milk or formula, and after 6 months, small sips of water between coughing fits.
  • Keep the room at 19-20 °C and humidify the air with a humidifier in the room or a bowl of water, Santé.fr advises.
  • Air out the room every day, and never smoke at home. Smoke alone is enough to irritate the throat.

How can you calm a coughing fit quickly?

To ease a coughing fit, pick up your baby. Sitting them upright or half-sitting helps them breathe. If it's a “barking cough,” Santé.fr advises staying with them in a closed bathroom filled with steam. If they have trouble breathing, call 15 (France's emergency medical number) or 112.

Can you give your baby cough syrup or other cough medicine?

No. Under age 2, cough suppressants and mucus thinners are contraindicated, according to France's national health insurance (Assurance maladie) and Santé.fr. With a wet cough, a cough suppressant could even trap mucus in the bronchial tubes.

Babies can't be treated like adults: adult cough syrups and codeine, which is contraindicated before age 12, are out of the question.

Before using any over-the-counter product, ask a doctor for advice, whatever your child's age. To treat a cough, the doctor addresses the cause if needed. An antibiotic does not work on a viral cold.

Does acetaminophen (Tylenol) calm a cough?

No. Acetaminophen (Tylenol) lowers fever and eases pain. France's national health insurance (Assurance maladie) recommends it if fever goes above 38 °C, not to suppress the cough reflex itself.

Should you raise the mattress when your baby coughs?

No. Some brand websites suggest putting a pillow under the mattress. Don't do it. The American Academy of Pediatrics (AAP) says that before age 1, babies sleep on their back, on a firm, flat mattress. A surface tilted more than 10 degrees is not safe, even with reflux. Raising the head at night is advised for adults by France's national health insurance (Assurance maladie), not for babies.

When they're awake, hold your baby upright, then lay them back down flat on their back.

How long does a baby cough last?

Children don't have a fixed cough duration: if your child has coughed for more than 8 days without fever, France's national health insurance (Assurance maladie) advises seeing a doctor within the next few days. Most infection-related coughs fade on their own. In adults, a cough typically lasts two to three weeks.

When should you see a doctor if your baby coughs?

Before 3 months, see a doctor the same day, no matter what. After that, it depends on age and other signs, according to France's national health insurance (Assurance maladie):

AgeSee a doctor the same day
Under 3 monthsAs soon as they start coughing
3 months to 2 yearsWith fever, trouble drinking or breathing, diarrhea, or a rash
Over 2 yearsChange in behavior, or moderate fever for more than 3 days

If your child's cough comes with wheezing, Santé.fr also advises seeing a doctor the same day. Wheezing is a sign the doctor checks for, including bronchiolitis, which needs watching at night. In that case, children may need respiratory physiotherapy, if the doctor prescribes it.

Which signs mean you should call 15 or 112?

Call 15 (France's emergency medical number) or 112 if your child has trouble breathing: mouth open, nostrils flaring, or the neck caving in with each breath. Santé.fr says not to wait until their skin turns blue. Other emergency signs in children:

  • a faint or hoarse voice, or a barking cough with trouble swallowing;
  • their overall condition getting worse;
  • a sudden, explosive coughing fit right after choking.

What should you do if your baby coughs after swallowing a foreign object?

Call 15 (France's emergency medical number) or 112 right away. France's national health insurance (Assurance maladie) and Santé.fr warn that a toy or piece of food may have entered the airways, even if your baby seems better afterward.

What does your baby's breathing reveal on nights they cough?

No device replaces a doctor's advice. Breathing faster than usual is one of the signs that call for a doctor visit. You still need to know your baby's usual pattern, since normal rates change with age (normal breathing rate at each age).

A contactless sensor can help. The Mothair mattress pad, placed under the sheet, tracks your baby's breathing rate, heart rate, and movements night after night. It compares them with your baby's own 7-day baseline. If they breathe faster than usual or sleep more restlessly, that is worth describing to your pediatrician.

What it doesn't do: it records no sound, doesn't diagnose the cause, and doesn't replace a doctor visit. If you see a sign of an emergency, call 15 (France's emergency medical number) or 112 right away. Don't wait for the app.

Baby cough: the essentials

To ease the cough safely: saline nose rinses, fluids, a comfortable room temperature, no cough syrup, and call 15 (France's emergency medical number) or 112 at the first sign of distress. Details below:

  • Most often, it's a winter cold that gets better on its own.
  • Saline nose rinses, frequent fluids, and a room at 19-20 °C. These are the steps that help.
  • No cough syrup or mucus thinners before age 2.
  • Your baby sleeps flat on their back, even with a cold.
  • Under 3 months, see a doctor the same day.
  • Trouble breathing, a faint voice, or a swallowed object: call 15 (France's emergency medical number) or 112 right away.
